Position Summary

The Support Worker role involves delivering Community Access support according to clients' NDIS plans. The primary focus is assisting clients in safely participating in community activities, attending appointments, and promoting their independence and overall wellbeing.

Key Responsibilities

• Deliver person-centred Community Access assistance following each client's NDIS goals and support plan
• Facilitate attendance at appointments, social gatherings, shopping trips, recreational, and other community engagements
• Assist clients to safely engage in the community while encouraging their independence and personal choices
• Provide transportation and travel support as necessary for community access and appointments
• Promote clients' development and maintenance of social connections and involvement in activities aligning with their interests
• Offer support with daily living tasks during community activities when needed
• Ensure client safety and wellbeing during community participation
• Keep precise progress notes and records meeting Point Care standards
• Communicate effectively with clients, families, coordinators, and care team members
• Adhere to individual support plans, risk management protocols, health and safety guidelines, and organizational policies
• Uphold professional boundaries, client privacy, dignity, rights, and autonomy
• Engage in ongoing training and professional development to maintain compliance and provide top-quality support

Key Requirements

• Hold at least a High School Certificate or an equivalent qualification
• Possess valid Blue Card, Yellow Card, First Aid, and CPR certifications
• Have a current Police Check issued within the last three months
• Maintain a valid Open Driver’s Licence; international licenses accepted for eligible temporary visa holders as per Queensland regulations
• Own a reliable, registered vehicle with current comprehensive insurance
• Certified or working toward Certificate III/IV in Individual Support (Disability, Aged Care or related fields); enrollment proof required before starting
• Previous paid or personal experience supporting individuals with disabilities
• Reside in or near Hervey Bay at the time of application
• Experienced in providing Community Access support including assistance with appointments, outings, shopping, social and community participation
• Strong grasp of person-centred care, client independence, choice, and NDIS objectives
• Good knowledge of the NDIS and applicable Practice Standards
• Sound understanding of health and safety and duty of care obligations
• Ability to assist diverse clients with varying needs, backgrounds, communication methods, and abilities
• Excellent communication and interpersonal skills fostering positive and professional relationships
• Well-organized with aptitude for detailed record-keeping and progress documentation
• Basic proficiency in computer use including Microsoft Office and digital support platforms
• Committed to completing mandatory training and professional development as required
