Job Overview

This role involves assisting in collecting patient and family biodata through interviews and providing social information to Medical Social Workers to support patient care management. The position requires independent handling of routine cases and supporting various social work functions including casework, group work, community involvement, program development, and contributing to safe care initiatives.

Key Responsibilities

• Conduct case assessments for discussion with Medical Social Workers and recommend suitable interventions.
• Carry out care needs assessments through field visits.
• Support and assist in conducting group work sessions targeting specific groups.
• Help perform community needs assessments and engage in community mapping activities.
• Co-organize segments of community events.
• Plan and coordinate social work projects and programs.
• Contribute to maintaining and enhancing safe care standards, including quality improvement projects and adherence to ethical codes and hand hygiene protocols.

Educational Qualifications

Applicants should have completed GCE 'O' Level, GCE 'A' Level, or hold a Diploma qualification.
