About the Role
Gloucestershire County Council is seeking a dedicated Referral Taker to join the Hospital Discharge and Assessment Team on a permanent full-time basis. Located at Shire Hall, Block 5, 2nd Floor in Gloucester, this position involves being the primary contact point for incoming referrals. You'll assess and triage the information, work collaboratively with partner agencies, and take detailed, holistic referrals.
The role covers both the Front Door and Discharge to Assess (D2A) pathways and requires close cooperation with Care Navigators and wider referral takers across localities. It offers flexible working arrangements combining office and remote work, supporting teams across the county.
Candidate Profile
The ideal candidate must demonstrate excellent communication and teamwork skills, maintaining composure in busy settings while staying positive. A comprehensive approach is needed to log accurate referrals and client records, correctly prioritizing urgency to ensure timely, effective support. Signposting and coordinating with other agencies while sustaining professional relationships is essential.
Strong organizational abilities and adaptability to shifting priorities are required. Experience in customer-facing roles and proficiency with IT systems are essential. Familiarity with Liquid Logic (LAS) and other health databases is advantageous, though training will be provided.
