About the Role

Plymouth City Council is seeking a dedicated Community Outreach Worker to join their team, providing vital support to adults with various needs. The role focuses on fostering independence, enhancing health and wellbeing, and improving the quality of life for service users by connecting them with community resources and services. Key Responsibilities

  • Assist in the care management process by supporting adults with diverse needs.
  • Encourage development of wider community connections for service users.
  • Provide research, advice, and information to facilitate access to relevant local and wider community services and agencies.
  • Support individuals in improving their overall quality of life through engagement and signposting.

Candidate Requirements

  • Highly motivated and organized individual with excellent interpersonal skills.
