Job Overview

The role focuses on ensuring uninterrupted and effective patient care by managing medical services and addressing individual patient needs thoroughly.

Key Responsibilities

  • Engage proactively with clients to encourage wellness and preventive health practices, including education on regular screenings, immunizations, and healthy living.
  • Work collaboratively with healthcare professionals to develop tailored care plans emphasizing wellness and disease prevention.
  • Conduct health evaluations and screenings to detect health risks early and suggest preventive strategies.
  • Organize screenings for illnesses such as hypertension, diabetes, cancer, and infectious diseases as per patient or community requirements.
  • Provide relevant resources and information on wellness programs and preventive services.
  • Manage referrals to specialists and preventive health services based on client assessments.
  • Maintain clear communication among healthcare providers to enhance client care continuity and follow-up procedures.
  • Act as the central liaison between clients, healthcare providers, and other medical professionals.
  • Ensure effective communication, accurate documentation, and smooth care transitions for patients.
  • Monitor clients' adherence to prescribed preventive care plans, including screenings and interventions.
  • Follow up with clients to overcome obstacles, offer support, and reinforce the importance of preventive health measures.
  • Establish connections with community partners to broaden access to preventive care resources and services.
  • Keep detailed and accurate records of client care interactions, plans, and outcomes.
  • Prepare reports and documentation necessary for healthcare compliance and to assess the effectiveness of wellness and prevention initiatives.
  • Display flexibility to work across multiple sites as required.
  • Perform additional duties aligned with the wellness objectives of the organization.

Qualifications and Skills

  • Comprehensive knowledge of preventive care principles including screenings, vaccinations, lifestyle interventions, and health promotion.
  • Stay informed about the latest guidelines and best practices in wellness and preventative care.
  • Exceptional communication and interpersonal skills to engage effectively with patients, healthcare providers, and other stakeholders.
  • Ability to explain complex medical information in clear, understandable terms and establish rapport with diverse clients.
  • Experience in care planning and coordination, including client needs assessment and interdisciplinary service coordination.
  • A clear understanding of healthcare system navigation and the importance of continuity of care.
  • Ability to motivate clients and educate them on the benefits of preventive health practices.
  • Strong organizational and time management skills to handle multiple cases, prioritize tasks, and ensure follow-up.
  • Capability to function effectively within established protocols and guidelines, managing multiple responsibilities.
  • Competence in data collection and interpretation relevant to wellness programs to aid decision-making and improve care delivery.
  • Cultural sensitivity and awareness to provide equitable, client-focused care respecting diverse backgrounds.
  • Proficiency in using electronic health records, care coordination tools, and related healthcare technologies.
  • Good understanding of health information privacy principles.
  • Ability to collaborate effectively within multidisciplinary teams and community entities to support care coordination and wellness promotion.
  • Strong teamwork skills fostering cooperation toward shared healthcare objectives.

Experience and Education

  • Minimum of three years’ experience as a clinical coordinator or in a similar capacity within healthcare settings.
  • Bachelor’s degree in nursing, public health, health education, healthcare administration, or a related healthcare field.
  • Additional certifications or training in care coordination, client engagement, or wellness program management are preferred.

Language and Licensing

  • Fluency in English, both spoken and written; Arabic proficiency is preferred.
  • Possession of valid registration and classification from the Saudi Commission for Health Specialties is required.
