Full-Time Case Manager

Role Overview

CBH Care is hiring a full-time Case Manager to support consumers in both community and on-site settings in Westwood, New Jersey. In this role, you will help carry out treatment plans, coordinate case services, assist with transportation and referrals, and make sure all required records and paperwork are completed accurately.

Key Duties

• Deliver direct case management and hands-on support to consumers at the program site and in the community.
• Carry out treatment plans and manage assigned cases, including transportation assistance and service linkages.
• Maintain complete and accurate documentation for all case-related activities.
• Work as part of the team by helping with shift coverage, taking part in program activities, and coordinating with coworkers to ensure all responsibilities are covered.

Qualifications

• Bachelor of Social Work (BSW), or a BA/BS in psychology, or a similar related field.
• At least 2 years of experience in mental health and human services.
• A valid driver’s license is required.

Schedule and Location

This is a full-time, on-site role based in Westwood, New Jersey. The schedule is either Sunday through Thursday or Tuesday through Saturday, with 3 day shifts and 2 evening shifts.

Compensation

The annual salary for this position is $40,000 to $43,000.
